Welcome to the Hudson Valley Collective for Community Wellness!


This page offers you all the information you need to become a member of the HVCCW!  Did you know that we have two types of memberships in our Collective?  That’s right, we have CBO Members who

  • have less than a $5 Million operating budget
  • are non-Medicaid Billing
  • are non-profit
  • address at least one Social Determinant of Health

For organizations who do not meet this criteria we welcome you as Alliance Members to the Collective.

We have two Community Based Organization (CBO) HUB’s in the Hudson Valley Region.  Each HUB has a coordinator.  The contact information for each coordinator is listed below:

Resources To Support Membership!


Here are some helpful resources for your review:

HVCCW Membership Survey:  We are asking all members to complete this survey (it’s about 20 minutes) to help us better understand the services you provide, the resources you offer, and how the HVCCW can help you with your mission work through capacity building, training, and readiness in the health transformation environment.

Meeting Minutes and Trainings:  The Collective meets once a month, usually on line, and meeting minutes and events are recorded and shared with Members.  Check out a few of our postings to get an understanding of our common agenda!

Policy and Procedures:  Here you can learn more about the HVCCW operations by reviewing Collective policies and procedures.

Memo of Understanding:  This document outlines your organizations understanding of membership requirements.  A signed MOU is required by each member organization in order to be eligible for the participation reimbursement.  In addition to the MOU each member needs to submit their operating budget, most recent 990, and most recent audit.  By submitting this information you will be eligible for a participation reimbursement from the HVCCW.  Membership paperwork can be signed and sent to your regional coordinator (contact information listed above).
