VBP Group Activity Worksheet:  What would be Your Value Statement?.  This document was used both pre and post conference by CBO’s to help develop an organizational values statement.  Download this worksheet today and draft your values statement for use in the VBP environment.

New York State Department of Health Survey.  Over the past two years, the NYS Department of Health (Department) has provided planning grant dollars to two consortiums to support non-Medicaid billing CBOs in engaging with providers, Performing Provider Systems (PPS) and Managed Care Organizations (MCO).  Your organization has been identified as a member of one of these consortiums.  The Department would appreciate your assistance in completing this 4-minute survey to gauge the impact of the consortium efforts.

Community Based Organization Survey.  The purpose of this survey is to capture current services your organization provide that addresses Social Determinants of Health (SDH) and gauge CBO integration with the New York State VBP program. The goal is also for the State to learn about SDH initiatives to share with the community at large, and to use the information provided to build a public inventory of Tier 1, 2, and 3 CBOs that can be used to facilitate VBP contracting. CBOs will be asked to complete this survey on an
annual basis.
